An invoice is the document that asks to be paid. It names both parties, gives the work a reference and a date, lists what was done and at what price, and says when payment is due. Without those, it is a receipt for a conversation.

The download is a real Word document, not a text file with a heading. Google Docs opens it natively, which is why this is a .docx rather than anything else.
1. The header
INVOICE across the top with a rule underneath, then your details on the left and the number, issue date and due date on the right, laid out with an invisible table so the two blocks stay side by side after Docs converts it.
2. The line items
One row per piece of work with the quantity, the rate and the amount, in a table with a yellow header row that repeats if the invoice runs onto a second page.
3. The totals
Subtotal, tax at whatever rate you set, and the total in bold with the currency code beside it. The tax line is left out entirely rather than showing zero when the rate is zero.
4. Notes and terms
Whatever you put in the notes field, which is where payment terms belong, followed by a small line naming where the document came from.
Everything is editable once it is open. This produces a filled in starting point rather than a locked document, which is the difference between a template you fight and one you use.

Google Docs
Upload it to Google Drive and open it, or use File then Open inside a Docs document. Docs converts the Word file on the way in and keeps the heading, the table and the alignment.
Microsoft Word
Double click the download. If Word shows a yellow protected view bar, click Enable Editing before you change anything.
Pages on a Mac
Open it directly. Pages converts the document and keeps the table, though the yellow header row may shift slightly.
